Log in to My Journey / My Professional Center and select student. (The student may be called Customer, Client, or Resident depending on your configuration.)
Click on the Assessments workflow step and click on Add Assessment.
If the appointment was a walk-in, chose that appointment type, enter the date, assessment and the score and click Save.
If you're uploading a file, click on the Choose File, navigate to the file, select it and click open. Once you see the file name listed in the field, click on Upload Assessment and you should see the file listed and be able to View or Print.
