Log in to CommunityPro Suite and find and select student by searching from the Students tab. Note: this tab may be called Clients, Customers, Participants or Students depending on how your agency or organization refers to its participants.
Enter the Transition Specialist workflow by clicking on the handshake icon beneath the student's name and id.
The Needs Inventory & Next Steps (previously called Transition Planner) has several tabs on which to capture the needs and current status of the students. You can save it after different meetings or to capture changes; each time it is saved, it will have a timestamp and will be available to read.
Select the tabs provided in the workflow. Note: Users may not see all tabs and those they see may be named differently as this is a configurable feature determined on a per client basis.
User selects Current Education to identify and select what program(s) the client is enrolled in.
User selects College Plans to identify and select client's college plans and interests.
User selects Education Services to identify and select what education services would be helpful for the client.
User selects Employment Services to identify and select what employment services would be helpful for the client.
User selects Financial Assistance to identify and select what kinds of financial assistance would be helpful for the client.
User selects Family & Community Engagement to identify and select what family and community engagement areas would be helpful for the client.
User selects Health and Human Services to identify and select what kinds of health and human services programs or support would be helpful for the client.
Once all desired options have been selected, user clicks on the Save Inventory button to create the Needs Inventory.
User reviews and can print the client's Needs Inventory by clicking on the Magnifying Glass under the Actions column under the Needs Inventory section.
User adds contact information and documents contact made and information about receiving agency and services student is receiving under Referral History using the '+' to Add a Referral Followup and the pencil on paper icon to Edit Referral Followup accordingly to document contact made with student.
User adds additional notes for the student under the Notes section using the Add and Edit buttons accordingly.
